Your first appointment
On the day of your first appointment we ask that you arrive a few minutes early to fill out your New Patient Forms. If you prefer, download them here , fill out only the sections outlined in GREEN, and bring them with you to your appointment or email your completed forms to firstname.lastname@example.org. Please bring a photo ID, insurance card if you have one and any relevant health information with you: test results, MRI reports, current medication lists, etc.
On the day of your appointment, you will be greeted quickly and provided a tour of our office. The doctor will then meet with you to discuss your health concerns and to confirm that you are in the right place for your best results. If you would like to proceed, a non-invasive exam will be performed and x-rays taken if warranted. You will not be asked to disrobe and the whole visit should take less than 45 minutes. At this point, any necessary testing or panels will be scheduled and your second visit will be booked.
The doctor will compile the exam findings and provide you with a recommended plan of action on your next visit. Your personalized care program will begin at that time. We will explain insurance and financial obligations then as well.
We accept all forms of insurance, and indeed most insurance offers some coverage for chiropractic care. We are classified as an out-of-network provider and require payment at the time of service, however our office will happily verify the amount your insurance should contribute and we will even submit the claim for you. A common misconception, out-of-network does NOT mean no insurance coverage. In fact, in many cases, the difference between in-network and out-of-network providers is minor, particularly concerning chiropractic care. Our "out-of-network" classification allows the doctor and the patient - not the insurance company - to decide the best care for you and allows more flexibility in payment options, notably with student and family plans. Any payments from insurers will be sent directly to you in a timely manner.
If you don't currently have insurance, we offer reasonable cash or credit options.
We are an in-network Medicare provider. Medicare generally covers chiropractic adjustments. You are responsible for the initial exam, x-rays and any deductibles or co-insurance. If you have secondary or supplemental insurance you may be reimbursed for these fees.

Auto or work Accidents
If you have been injured in a motor vehicle accident or at work, you are entitled to Chiropractic and massage services to bring you back to health. Please contact us immediately.
Payment for care is due at the time of service. We accept:
• Cash or Check
• Flexible Spending Accounts and/or Health Savings Accounts
• Individual Care Plans. These offer multiple payment options with significant discounts.
• Family Care Plans. Available to immediate family members in the same household.
